Will it re-add decisions that are already logged?
No — it dedupes against the existing log. A decision that gets restated in a later meeting strengthens the existing entry rather than creating a duplicate.
What happens when leadership reverses itself?
The reversal is flagged, not papered over: the new decision is logged with a pointer to the earlier one it contradicts, so the team can see what changed and when.
What tools does it need?
